How the program may use and disclose protected health information about an individual: 

o To collaborate with other providers or professionals you work with 

o To manage operations and run our programs, improve the quality of your care, and stay in communication.

Client rights with respect to the information and how the individual may exercise these rights, including how the individual may complain to the program: 

o Access to your medical record and request amendments or corrections 

o Contact us using various means of confidential communication 

o Request limits to, and generate a list of recipients of, releases of information and receive a historical account of who has received information 

o Right to exercise the bounds of power of attorney and/or legal guardianship 

o Submit requests to change the boundaries of your release of information
